The forms linked below are official documents from the New York State Unified Court System, the Division of Housing and Community Renewal (DHCR), and other government agencies. All links direct you to official government sources—this site doesn't host modified versions.

Tenant Forms

Answer Form (CIV-LT-91a)

"The Shield"

Purpose: Your formal written response to an eviction petition. This is where you raise your defenses—improper service, repair problems, rent overcharges, retaliation—and preserve your right to fight the case.

Deadline: 5 days for non-payment cases; 10-12 days for holdover cases (check your papers)

Affidavit to Vacate Default Judgment

"The Reset Button"

Purpose: If you missed your court date and a default judgment was entered against you, this form asks the court to undo ("vacate") that judgment and give you a new hearing. You'll need to show a reasonable excuse for missing court and a valid defense to the case.

Poor Person's Relief (Fee Waiver Affidavit)

"The Money Saver"

Purpose: Request that the court waive filing fees if you cannot afford them. Courts grant fee waivers based on income and financial hardship.

14-Day Rent Demand

"The Warning Shot"

Purpose: The required first step before filing a non-payment eviction. This written notice tells the tenant exactly how much rent is owed and gives them at least 14 days to pay before you can file in court. Improper rent demands can get your case dismissed.

Non-Payment Petition (UCS-LTN1, UCS-LTN2)

"The Opening Move"

Purpose: The court filing that officially starts a non-payment eviction case. UCS-LTN1 is the Notice of Petition; UCS-LTN2 is the Petition. Both must be properly served on the tenant.

Holdover Petition (UCS-LTH2)

"The Eviction Petition"

Purpose: The court filing for evictions based on reasons other than non-payment—lease violations, lease expiration, nuisance, owner's use, or other grounds. Holdover cases require proper predicate notices before filing.

Affidavit of Service

"The Proof"

Purpose: Sworn statement proving that court papers were properly served on the other party. Required for all Housing Court filings. The person who served the papers (not you) must sign this form, and it must be notarized.

Lease Compliance & Rent Stabilization Forms

Renewal Lease Form (RTP-8)

"The Renewal Ritual"

Purpose: The official DHCR form landlords must use to offer lease renewals to rent-stabilized tenants. Must be sent 90-150 days before the current lease expires.

Annual Rent Registration (RA-LR1)

"The Yearly Report Card"

Purpose: Landlords of rent-stabilized buildings must register all apartments and their legal rents with DHCR annually by July 31. Failure to register results in a rent freeze.

Bedbug Disclosure Notice (DBB-N)

"The Bug Report"

Purpose: Mandatory disclosure of a building's bedbug history for the past year. Must be provided to prospective tenants before they sign a lease and to current tenants with their annual lease renewal.

EPA Lead Disclosure Form

"The Safety Check"

Purpose: Federal law requires landlords to disclose known lead-based paint hazards before renting housing built before 1978. Both landlord and tenant must sign this form.
